New Patient Paperwork

It will acquaint you about who we are, what we do, and how we’re different from every other clinic in the area.   It will explain our unique approach to care, the kinds of conditions we treat, financial information, how to find our office, and other key points.  The answers to most of your questions can be found right here.  If you have any questions whatsoever please feel free to call us at your earliest convenience.  We’re here to help. In order to serve you better and reduce the time you have to spend in the clinic, we ask that you handle your New Patient paperwork BEFORE your first appointment with the doctor.   If you can have the paperwork finished before you come to your appointment, when you arrive, we can go straight to your consultation and examination.

 
Step 1:  Download the forms . (You'll need Adobe Acrobat Reader.)
Step 2:  Print the forms on your printer.
Step 3:  Fill out the forms (as completely as you can).
 
When you're finished, you should have five (5) pages completely filled out.  Please make sure to sign and date the bottom of each form.  Please bring the New Patient paperwork forms with you to your first visit along with a Photo ID and your Insurance Card.  We look forward to serving you soon!
